    The Kent and Medway Integrated Care System is a partnership of organisations that
    come together to plan and deliver joined up health and care services to improve the lives
    of people across Kent and Medway.

    We have four key purposes:
     improving outcomes in population health and healthcare;
     tackling inequalities in outcomes, experience and access;
     enhancing productivity and value for money;
     supporting broader social and economic development.

    Who is included?

    Primary Care
    • Primary care services provide the first point of contact in the healthcare system,
    acting as the ‘front door’ of the NHS. Primary care includes general practice,
    community pharmacy, dental, and optometry (eye health) services.
    Primary care networks include groups of general practices working together, and
    in partnership with community, mental health, social care, pharmacy, hospital and
    voluntary services in their local area, to offer more personalised, coordinated
    health and social care to the people living in their area.
    • There are around  40 PCNs in Kent and Medway. Networks are aligned to
    health and care partnership – this supports them to work more closely with
    other partners in the area.

    Secondary Care

    Secondary care includes:
    • planned or elective care – usually in a hospital
    • urgent and emergency care, including 999 and 111 services, ambulance
    services, hospital emergency departments, and out-of-hours GP services
    • mental health care
    Community Health
    Community Health Services Include:
    • district nursing
    • health visiting
    • child health services
    • sexual health services
    • and many others

    Social Care

    Social Care is an important element of the Health and Care Pathway. In Kent and
    Medway, Social Care is managed through the two councils, Kent County Council and
    Medway Council.
    What is the difference between healthcare and social care?
    A healthcare need is related to the treatment, control or prevention of a disease, illness,
    injury or disability, and the care or aftercare of a person with these needs.
    A social care need is focused on providing assistance with:

    • the activities of daily living
    • maintaining independence
    • social interaction
    • enabling the individual to play a fuller part in society
    • protecting them in vulnerable situations
    • helping them to manage complex relationships

    and (in some circumstances) accessing a care home or other supported accommodation

    Apprenticeships

    What you need to know about Health and Care apprenticeships in Kent and Medway:

    • if you are aged 16 to 18 or 19+ and in your first year of an apprenticeship, the
    minimum you should receive is £4.81 an hour (the National Minimum Wage for
    apprentices)
    • many employers will pay you more than the minimum wage
    • they take between one and five years to complete
    • anyone over the age of 16 can do one
    • you’re likely to spend four days on work placement and one day or at a training
    centre or college
    • you’ll develop your skills, including English and maths
    • you’ll gain a competence qualification (based on what you can do in the workplace)
    and a knowledge qualification, or a qualification combining both
    Apprenticeships are available in all sectors across Primary, Secondary,
    Community, Social and Tertiary Care.

    Apprenticeship levels

    • NHS apprenticeships are available at several levels:
    • level 2 – equivalent to GCSEs
    • level 3 – equivalent to A-levels
    • levels 4 and 5 – equivalent to a foundation degree and above
    • levels 6 and 7 – equivalent to a full bachelor’s or Master’s degree
    Entry requirements
    • Entry requirements will depend on the employer and the type and level of
    apprenticeship.
    • For example, you may need four or five GCSEs at grades 9-4/A-C or equivalent to
    do a level 3 apprenticeship. To start a level 5 or 6 apprenticeship you're likely to
    need A-levels, T-Levels, equivalent level 3 qualifications or relevant and sufficient
    experience.
